Psychotherapy codes payers won't pay together
Most therapy claim denials aren't about whether therapy happened. They're about which codes went on the claim together. Here are the pairings that cause them.
Pick one session code per day
The individual psychotherapy codes are chosen by the time spent with the client: 90832 for 16 to 37 minutes, 90834 for 38 to 52 minutes and 90837 for 53 minutes or more. They describe the same service at different lengths, so a payer pays only one of them for the same clinician on the same day. If you saw a client twice in one day, add the time together and bill the code that fits the total, unless the payer tells you otherwise.
The intake already includes the therapy
The diagnostic evaluation, 90791 (or 90792 when it includes medical services), isn't paid on the same day as a psychotherapy session by the same clinician. Bill the evaluation for the intake day, and start the session codes at the next visit.
Crisis codes stand alone
Psychotherapy for crisis, 90839 for the first 30 to 74 minutes and 90840 for each further 30 minutes, replaces the regular session codes for that day. It isn't billed with 90832, 90834 or 90837, and the interactive complexity add-on 90785 isn't used with it.
Add-ons need their base code
- 90785 (interactive complexity) goes with a diagnostic evaluation, a session code or group therapy, never alone.
- 90833, 90836 and 90838 are psychotherapy added to a medical visit (an E/M code). They need that visit on the same claim.
- 96131 and 96137 are the additional hours of psychological testing and need 96130 or 96136 with them.
Who can bill what
Medicare doesn't let clinical social workers bill psychological testing (96130 to 96137) or 90792, and psychologists can't bill 90792 either. Neither can bill an E/M visit, so the psychotherapy add-ons 90833, 90836 and 90838 aren't theirs to use; they bill 90832, 90834 or 90837 instead. Many Medicaid plans and insurers follow the same lines.
